OBEDIENCE REGISTRATION FORM

Effective July 1, 2007, all Active SOTC Members will receive a $10 discount on each class.

You have two options to Register:

Download the Obedience Registration Form in PDF format, print the PDF and mail the completed registration form and a check payable to S.O.T.C. to:

Download the Obedience Registration Form in a Microsoft Word document format. Complete the form and return via email to Leslie@sotcdogtraining.com. Then print the document and mail the completed registration form and a check payable to S.O.T.C. to:

PLEASE NOTE: Before sending form & check, YOU MUST call Leslie Morton @ 941-374-SOTC (7682) or email Leslie@sotcdogtraining.com to check class space availability.

Classes fill as payment is received by the registrar. Most classes fill 2-3 weeks before class starts.
For everyone's benefit, we limit the maximum number of students in a class. Most classes have only 8 students, so classes will fill quickly.
Very rarely do we not meet the minimum number of students (4). If this happens, you will be notified 1 week before class starts and your money will be refunded.
Registration closes 24 hours before class starts. If you need to change or withdraw from a class, it must be done before registration closes. There will be no refunds after this time.
Your signature on the Obedience Registration Form verifies that you have read, understand and will comply with the OBEDIENCE CLASS POLICIES.
