OBEDIENCE REGISTRATION FORM

Effective July 1, 2007, all Active SOTC Members will receive a $10 discount on each class.

How to Register:

Download the Obedience Registration Form, which is a Microsoft Word document.. To complete the form on your computer, simply open the file using Microsoft Word or any other word processing program, place your cursor on a red line, click and begin typing. You should have enough space to include your name and contact information. If the information flows over to another line, that's ok. As long as everything is there, we can read it. Please be sure to supply your phone number so that we can call you with any questions. After your registration form is completly filled out, click to save and rename the file to "yourname.doc". You will then send an email to Obedience@sotcdogtraining.com and select "add attachment". Browse to find your newly saved file, click to add as an attachment and click to send to Leslie. To complete the registration process, print the document, insert into an envelope along with a check payable to S.O.T.C. for the class fee and mail to:

If you do not want to complete the form using your word processing program, you may click HERE to download a PDF form that you can print, fill out and send in to the above address with the appropriate class fee. This form has plenty of space for your handwritten contact information.

PLEASE NOTE: Before sending form & check, YOU MUST call Leslie Morton @ 941-374-SOTC (7682) or email Obedience@sotcdogtraining.com to check class space availability.

Classes fill as payment is received by the registrar. Most classes fill 2-3 weeks before class starts.
For everyone's benefit, we limit the maximum number of students in a class. Most classes have only 8 students, so classes will fill quickly.
Very rarely do we not meet the minimum number of students (4). If this happens, you will be notified 1 week before class starts and your money will be refunded.
Registration closes 24 hours before class starts. If you need to change or withdraw from a class, it must be done before registration closes. There will be no refunds after this time.
Your signature on the Obedience Registration Form verifies that you have read, understand and will comply with the OBEDIENCE CLASS POLICIES.
