Important information! Please read before downloading the registration form.
- Your signature on the Flyball Registration Form verifies that you have read, understand and will comply with the FLYBALL CLASS POLICIES.
- You MUST contact the Flyball Registrar to check class space availability BEFORE sending in your registration form and check, in order to get your name on the registration "pending" list for a class.
- If a class does not meet the minimum number of students by one week before the starting date, the class may be cancelled or postponed. You will be notified if this happens. Checks will be returned if a class is cancelled.
- There is a retuned check fee of $40.
- NO REFUNDS will be made after the first night of class.
