AGILITY REGISTRATION FORM
Important information! Please read before downloading the registration form.
- Your signature on the Agility Registration Form verifies that you have read, understand and will comply with the AGILITY CLASS POLICIES.
- You MUST contact the Agility Registrar to check class space availability BEFORE sending in your registration form & check, in order to get your name on the registration "pending" list for a class.
- Pre-registration and payment is required ONE WEEK before the class start date. Registration forms submitted within one week of the class start date will be returned. NO registration forms will be accepted the first night of class.
- If a class does not meet the minimum number of students by one week before the starting date, the class may be cancelled or postponed. You will be notified if this happens. Checks will be returned if a class is cancelled.
- There is a retuned check fee of $40.
- NO REFUNDS will be made after the first night of class.
