Important information! Please read before downloading the registration form.
- Your signature on the Agility Registration Form verifies that you have read, understand and will comply with the AGILITY CLASS POLICIES.
- NOTE: you MUST check with the Agility Registrar for class availability before you send in your registration form. If your payment and registration form is not received ONE WEEK prior to the class start date, you will not be guaranteed a spot in a class. NO registration forms will be accepted the first night of class.
- If a class does not meet the minimum number of students by one week before the starting date, the class may be cancelled or postponed. You will be notified if this happens. Checks will be returned if a class is cancelled.
- There is a retuned check fee of $40.
- NO REFUNDS will be made after the first night of class.
